• Physical description: 21 lines per page; written in naskh in black on modern paper. Rubrication and catchwords; a few marginal corrections. A few smudges, but text still legible; otherwise in excellent condition.
• Origin: Copy completed by Muḥammad Ḥasan al-Ḥusaynī ibn Muḥammad Ismāʻīl al-Ḥusaynī al-Riḍawī al-Simnānī al-aṣl wa-al-Kirmānshāhī al-maskan on 8 Shawwāl 1313 H. [23 March 1896] (fol. 172a).
• Incipit: کتاب چهارم اندر استخراج امراض ىعنى شناختن هر بىمار ىکه کدام است و شناختن نضج و بحران و شناختن که حال بىمار چگونه خواهد شد و آنرا طبىبان تقدمة المعرفة گوىند و اين کتاب چهارم گفتار است
